and excellence Role Overview As a Hospice RN at Vital Caring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.
You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.
What You’ll Do Lead Patient-Centered Hospice Care Conduct comprehensive patient and family assessments Develop and manage individualized, comfort-focused care plans Deliver skilled nursing care with a focus on pain and symptom management Serve as the Care Coordinator Act as case manager, coordinating care across the interdisciplinary team Collaborate with physicians, social workers, chaplains, and aides Monitor patient condition and adjust care plans to maintain comfort Support Patients & Fa
