About the Role

Our client, a non-profit organization focused on improving access to essential services for underserved communities, is seeking a proactive and resourceful Trabajador Social de Enlace y Coordinación de Servicios . This role is essential for connecting individuals and families with the resources they need, including healthcare, housing, food assistance, and employment support. The position involves working remotely to build strong relationships with community partners, facilitate referrals, and provide advocacy for clients. Key Responsibilities

- Conduct needs assessments to identify barriers to accessing social services for individuals and families.
- Develop and maintain a comprehensive directory of community resources and referral agencies.
- Facilitate warm handoffs and referrals to appropriate services, ensuring follow-up for service completion.
- Provide information and education to clients about available social programs and eligibility requirements.
- Advocate on behalf of clients to overcome systemic barriers and ensure equitable access to services.
- Collaborate closely with partner organizations, government agencies, and community leaders to strengthen service networks.
- Maintain accurate client records, track referral outcomes, and generate reports on service coordination activities.




- Develop and deliver online presentations or webinars to educate community members on available resources.
- Stay informed about changes in social policies and program updates that may affect clients.
- Contribute to program development and improvement initiatives based on client needs and feedback.

Requirements

- Bachelor's degree in Social Work, Sociology, Public Health, or a related field.
- Minimum of 3 years of experience in case management, social services coordination, or community outreach.
- Demonstrated knowledge of social welfare programs, health services, and community resources in Chihuahua .
- Excellent communication, interpersonal, and negotiation skills.
- Strong organizational skills and ability to manage multiple client cases and referrals simultaneously.
- Proficiency in using case management software, databases, and virtual communication tools (e.g., video conferencing, email).
- Ability to work independently, prioritize tasks, and manage time effectively in a remote work environment.
- Strong advocacy skills and commitment to social justice.
