Key Responsibilities
- Provide comprehensive medication management services, including drug therapy optimization, patient education, and adherence monitoring, remotely.
- Collaborate with physicians, nurses, and other healthcare providers via telehealth platforms to ensure safe and effective medication use.
- Conduct medication reconciliation for patients transitioning between care settings, ensuring continuity of care.
- Develop and implement patient-specific pharmacotherapy plans, adapting them as needed based on patient response and clinical updates.
- Answer drug information queries from healthcare professionals and patients in a timely and accurate manner.
- Participate in interdisciplinary team meetings (virtual) to discuss complex patient cases and treatment strategies.
- Contribute to the development and refinement of remote patient care protocols and workflows.
- Stay updated on the latest clinical guidelines, drug therapies, and regulatory changes affecting pharmacy practice.
- Utilize electronic health records (EHR) and telehealth systems effectively.
- Mentor and support junior remote pharmacy staff as needed.
Requirements
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Active and unrestricted pharmacy license in Mexico.
- Minimum of 7 years of experience in clinical pharmacy, with a strong emphasis on direct patient care.
- Demonstrated experience or a strong aptitude for telehealth and remote patient management.
- Excellent understanding of pharmacotherapy, disease states, and drug interactions.
- Proficiency in using EHR systems and other digital health tools.
- Exceptional communication, interpersonal, and critical-thinking skills.
- Ability to work independently, manage time effectively, and maintain productivity in a remote setting.
- Experience with medication therapy management (MTM) programs is highly desirable.
- Bilingual proficiency (Spanish and English) is a plus.
