About the Role

Our client, a leading healthcare provider in Querétaro City, Querétaro, MX , is actively searching for a dedicated and knowledgeable Farmacéutico Clínico . This crucial role involves providing advanced pharmaceutical care within a hospital setting, focusing on medication therapy management, clinical interventions, and ensuring patient safety. You will collaborate closely with medical teams to optimize drug therapy regimens, minimize adverse drug events, and contribute to evidence-based practice. Key Responsibilities

- Conduct medication reconciliation for patients upon admission, transfer, and discharge.
- Participate in interdisciplinary rounds to provide drug therapy recommendations.
- Monitor patients for therapeutic effectiveness, adverse drug reactions, and drug interactions.
- Educate patients and healthcare providers on appropriate medication use.
- Develop and implement clinical pharmacy protocols and guidelines.
- Manage and dispense complex medications, including intravenous therapies.
- Participate in formulary management and drug use review activities.
- Provide education and mentorship to pharmacy residents and students.




- Contribute to quality improvement initiatives related to medication use.
- Stay updated on clinical practice guidelines and emerging pharmaceutical research.

Requirements

- Doctorate in Pharmacy (Pharm.D.) or equivalent from an accredited institution.
- Licensed Pharmacist in the state of Querétaro, Mexico.
- Completion of a PGY-1 Pharmacy Residency program is highly preferred.
- At least 3 years of progressive experience in clinical pharmacy, preferably in an acute care setting.
- Strong understanding of pharmacotherapy, clinical pharmacology, and evidence-based medicine.
- Excellent communication and presentation skills, with the ability to interact effectively with diverse healthcare professionals and patients.
- Proficiency in using electronic health records (EHR) and pharmacy information systems.
- Demonstrated ability to work collaboratively within a healthcare team.
- Board certification in a relevant specialty (e.g., BCPS, BCPP) is a strong asset.
