About the Role

Our client is seeking an empathetic and dedicated Gestor/a de Casos para Personas sin Hogar to join their outreach team in Chihuahua, Chihuahua, MX . This on-site position is critical in providing direct support, case management, and linkage to essential services for individuals experiencing homelessness. The idóneo candidate possesses a strong understanding of the complexities of homelessness, including mental health, addiction, and socioeconomic factors. You will work closely with clients to develop personalized action plans, connect them with housing, healthcare, employment opportunities, and other vital resources. This role requires resilience, compassion, and a commitment to advocating for vulnerable populations within the community. Key Responsibilities

- Conduct outreach to individuals experiencing homelessness in designated areas.
- Perform comprehensive assessments to identify client needs, strengths, and barriers.
- Develop individualized case plans with clear goals for housing, employment, and support services.
- Assist clients in accessing emergency shelter, transitional housing, and permanent housing solutions.
- Connect clients with healthcare providers, mental health services, substance abuse treatment, and job training programs.
- Provide ongoing case management and follow-up to ensure clients are meeting their goals.
- Maintain accurate and confidential client records in accordance with agency policies and funding requirements.




- Advocate for clients' needs with various service providers and government agencies.
- Collaborate with community partners and stakeholders to coordinate services and resources.
- Respond to immediate client needs, including crisis intervention and emotional support.

Requirements

- Bachelor's degree in Social Work, Sociology, Psychology, or a related field.
- Minimum of 3 years of experience in case management, social work, or outreach, preferably with homeless populations.
- Demonstrated knowledge of homelessness issues, local resources, and support systems.
- Strong assessment, crisis intervention, and problem-solving skills.
- Excellent interpersonal, communication, and advocacy skills.
- Ability to work independently and effectively in challenging field conditions.
- Proficiency in using case management software and maintaining accurate records.
- Empathy, patience, and a non-judgmental approach.
- Valid driver's license and reliable transportation for outreach activities.
