About the Role

Our client is actively recruiting a dedicated Farmacéutico Clínico with specialized expertise in Insuficiencia Cardíaca to join their esteemed medical center in Querétaro City, Querétaro . This role is integral to our patient-centered care model, focusing on optimizing medication therapy for patients diagnosed with heart failure. The Farmacéutico Clínico will work collaboratively within an interdisciplinary Heart Failure program, providing direct patient care, medication management, education, and support to improve patient outcomes and reduce hospital readmissions. Key Responsibilities

- Conduct comprehensive medication therapy management (MTM) for patients with heart failure.
- Assess patient's medical history, disease state, and potential drug-related problems.
- Develop, implement, and monitor individualized pharmacotherapy plans in collaboration with physicians and advanced practice providers.
- Educate patients and caregivers on their medications, disease management, lifestyle modifications, and the importance of adherence.
- Monitor patient progress, adjust medication regimens as needed, and document interventions and outcomes.
- Participate in heart failure clinic rounds, multidisciplinary team meetings, and patient discharge planning.
- Identify opportunities to optimize guideline-directed medical therapy (GDMT) for heart failure patients.




- Contribute to the development and implementation of protocols and pathways for heart failure management.
- Engage in quality improvement initiatives to enhance heart failure care delivery.
- Provide education and mentorship to pharmacy students and residents.

Requirements

- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Valid and current pharmacy license in Mexico.
- Completion of a PGY-1 Pharmacy Practice Residency preferred, or equivalent clinical experience.
- Minimum of 3 years of clinical pharmacy experience, with a strong focus on cardiovascular pharmacotherapy and heart failure management.
- Board Certification in Pharmacotherapy (e.g., BCPS) or a commitment to obtain it within a specified timeframe is highly desirable.
- In-depth knowledge of heart failure pathophysiology, pharmacotherapy, and current treatment guidelines.
- Experience with anticoagulation management and other cardiovascular medications.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work effectively within a multidisciplinary healthcare team.
- Proficiency with electronic health records and pharmacy information systems.
- Strong organizational and time management skills.
