Summary

The position is located in the Ambulatory Care Clinic within the Division of Nursing at Crownpoint Service Unit. The facility provides primary care clinic/outpatient services to beneficiaries in the surrounding community. The primary purpose of this position is to serve as a Patient Care Coordinator/ Case Manager for the outpatient clinical services, including the satellite clinics.

A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

• Develop, implement, evaluate, and revise patient care coordination and case management programs while ensuring compliance with current standards of care, policies, and procedures.
• Coordinate comprehensive patient care by collaborating with physicians, nurses, social workers, physical therapists, case managers, public health, and social services to address patient needs, manage clinic and follow-up visits, maintain health care requirements,



and achieve timely, cost-effective outcomes.
• Facilitate patient care conferences, provide interpretation support when applicable, conduct chart reviews, and document care coordination activities to monitor effectiveness, continuity of care, and compliance with documentation standards.
• Collects relevant data for program evaluation, including service utilization, identified problems, and the status of resolutions.
• Assesses and monitors patient satisfaction to support program improvement and enhance the quality and effectiveness of care coordination services.
