The role centers on patient communication, scheduling, care coordination, and clinical documentation. The ideal candidate brings a clinical background, experience in telephonic or remote care settings, and comfort managing a high-volume caseload of 150+ patients across multiple priorities. Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU, cardiac step-down, CCU, telehealth, utilization review, or home health will be competitive. What matters most is clinical credibility, strong communication, and the ability to keep patients connected to their care plan.Key ResponsibilitiesPatient Communication & SchedulingManage patient scheduling, reminders, and follow-up workflows in EMR/EHR systemsConduct telephonic or remote patient outreach to reinforce care instructions and coordinate next stepsSupport patients navigating cardiology care pathways, including CHF management protocolsCare CoordinationTrack testing schedules, follow-up appointments, and remote patient monitoring workflowsCoordinate with the clinical team to ensure continuity of care across a caseload of 150+ patientsEscalate clinical concerns to the appropriate provider within scope (no diagnosing or prescribing)Documentation & ComplianceMaintain accurate and timely clinical documentation in EMR/EHR systemsEnsure all patient interactions and records meet HIPAA compliance standardsSupport billing documentation and insurance coordination as neededWho Will SucceedThis role is a strong fit for someone who is patient-first, organized under pressure, and genuinely comfortable on the phone.
