Key Responsibilities

• Student Case Management:



Manage an assigned caseload of students, ensuring individual needs, concerns, and support requirements are identified and appropriately addressed.
• Student & Family Support: Serve as a primary point of contact for students and families, providing guidance, follow-up, and support throughout the case management process.
• Case Coordination: Coordinate with academic, pastoral, counseling, learning support, and other relevant teams to ensure students receive appropriate and timely support.
• Individual Support Plans: Support the development, implementation, and monitoring of individualized action or support plans based on student needs.
• Monitoring & Follow-Up: Regularly monitor student progress, attendance, engagement, and identified areas of concern, ensuring appropriate follow-up and escalation when necessary.
