When someone depends on care, every call, schedule, and decision matters. A missed shift or delayed response can create stress for clients, families, and caregivers.

We’re looking for a reliable, organized, and proactive Case Client Coordinator who enjoys helping people, solving problems, and keeping things running smoothly. In this role, you’ll coordinate caregivers, manage schedules, respond to urgent needs, and make sure clients receive the care they need when they need it.

What You’ll Do

- Schedule and assign caregivers based on client needs, care plans, and availability.
- Handle inbound and outbound calls with clients, families, and caregivers.
- Manage call-offs, schedule changes, and urgent coverage needs.
- Find quick solutions when caregivers are unavailable or plans change.
- Manage referrals and respond to service inquiries.
- Keep client and caregiver records accurate and up to date in HHA Exchange.
- Monitor the Call Dashboard and support pre-billing tasks.
- Make sure scheduled services follow each client’s Plan of Care.
- Report incidents, concerns, and important changes to management.
- Work closely with the care team to maintain reliable service.




- Participate in team meetings and scheduled on-call rotations.

What You’ll Bring

- Experience in case coordination, caregiver scheduling, healthcare, home care, or administration.
- Strong organization, attention to detail, and follow-through.
- Excellent communication and problem-solving skills.
- Ability to stay calm and make sound decisions under pressure.
- Comfortable handling high call volumes and changing priorities.
- Experience coordinating schedules, managing coverage, or supporting clients.
- Familiarity with healthcare, scheduling, or workforce management platforms.
- Proficiency in Microsoft Office.
- HHA Exchange experience is a plus.
- Spanish fluency is a plus.
