About the Role

We are looking for a Senior Software Engineer - FHIR to help design, build, and evolve a healthcare interoperability platform responsible for securely retrieving patient records from EHRs and external healthcare systems.

You will work on systems that support Release of Information (ROI) services, including patient identity matching, record discovery, data retrieval, normalization, and downstream delivery.

This is an opportunity to work on real-world healthcare interoperability challenges, influence architecture and technical direction, modernize legacy integrations, and help expand a growing FHIR-based platform.

AI-assisted development is a core part of the engineering workflow, so we are looking for an engineer who actively uses AI coding tools while maintaining strong engineering judgment and ownership of production code.

What You'll Do

- Design and build backend services and APIs using FHIR R4 and related healthcare interoperability standards.
- Build interoperability workflows supporting patient identity matching, record discovery, retrieval, and delivery.
- Integrate with EHR vendors, health information networks, and other external healthcare systems.
- Develop and modernize services using C#, .NET, ASP.NET Web API, Entity Framework, and SQL Server .
- Work with RESTful APIs, event-driven architectures, and distributed systems.
- Own technical roadmaps for interoperability components.
- Use AI coding tools daily for development, debugging, refactoring, and codebase navigation.
- Review AI-generated code for correctness, security, reliability, and maintainability.
- Build automated unit, integration, and end-to-end tests.
- Lead code reviews and contribute to engineering standards.




- Troubleshoot third-party integrations and distributed systems.
- Mentor engineers on backend development and healthcare integrations.

Required Qualifications
- 6+ years of professional software development experience.
- Strong proficiency with C# and the Microsoft .NET stack .
- Experience with .NET Framework / .NET 6+, ASP.NET Web API, ASP.NET MVC, Entity Framework, and SQL Server.
- Direct experience with healthcare interoperability and FHIR-based integrations .
- Experience building and operating API-driven distributed systems.
- Strong SQL, relational data modeling, and data consistency knowledge.
- Experience integrating third-party systems with an emphasis on reliability, error handling, and observability.
- Experience with automated unit, integration, and end-to-end testing.
- Git and collaborative development workflow experience.
- AWS or Azure experience.
- CI/CD and DevOps knowledge.
- Active, daily use of AI-assisted development tools .
- Bachelor's degree in Computer Science or a related field, or equivalent hands-on experience.

Nice to Have
- Deep knowledge of FHIR resources, profiles, and implementation guides.
- SMART on FHIR and OAuth2.
- Epic, NextGen, Meditech, or other major EHR integration experience.
- HIPAA-compliant patient data handling experience.
- Asynchronous workflows, messaging, and retry semantics.
- AWS EC2, SQS, SNS, Lambda, and RDS.
- Technical project leadership.
- Agentic AI workflow experience.
