We’re looking for a
proactive and highly organized Care Coordinator
who is comfortable handling a high volume of calls, managing multiple priorities, and finding solutions quickly. In this fully remote role, you’ll communicate directly with patients, coordinate with healthcare teams, and help keep care moving smoothly.

What You’ll Do
- Coordinate patient care, appointments, follow-ups, and services.
- Handle a high volume of inbound and outbound calls professionally.
- Communicate with patients, providers, and internal teams to resolve issues.
- Identify problems quickly and find practical solutions on the spot.
- Follow up with patients to ensure needs are addressed and next steps are clear.
- Manage multiple patient cases and priorities in a fast-paced environment.
- Maintain accurate patient information, notes, and documentation.




- Escalate complex issues when needed and ensure timely resolution.
- Collaborate with clinical and administrative teams to provide a smooth patient experience.

What You Bring
- Experience in
care coordination, patient support, case management, customer service, or a similar role
.
- Experience handling
high-volume calls
in a call center, healthcare, customer service, or support environment.
- Strong problem-solving skills and the ability to
think quickly and find solutions on your feet
.
- Excellent English communication skills, both written and verbal.
- Strong organization and attention to detail.
- Ability to manage multiple conversations, tasks, and cases at once.
- Comfortable working with CRM, EMR, Google Workspace, or similar systems.
- A proactive, patient-focused approach and the ability to stay calm under pressure.
