Organization Profile: Centre for Chronic Disease Control (CCDC) is a not-for-profit research organization that generates and translates evidence to prevent and control non-communicable (chronic) diseases in diverse low- and middle-income settings. CCDC is recognized as a Scientific and Industrial Research Organization (SIRO) by the Department of Scientific & Industrial Research (DSIR), Ministry of Science and Technology, Government of India. It is also a WHO Collaborating Centre for Surveillance, Capacity Building and Translational Research in cardio-metabolic diseases and an ICMR Collaborating Centre of Excellence.

Project Brief: Multiple long-term conditions (MLTCs), defined as the co-existence of two or more chronic conditions in an individual, affect anestimated 25–40% of adults attending primary care in India.



Care systems organised around single diseases are poorly equipped tomanage them. Community Health Officers (CHOs), deployed at Ayushman Arogya Mandirs (AAMs), are the policy-designated firstpoint of clinical contact for non-communicable disease (NCD) care in rural India, yet no MLTC-specific training resource currently existswithin their curriculum.

The project entitled, “Co-Designing and Piloting a Digital Training Intervention to Strengthen Community Health Officers’ Capacity for Managing Multiple Long-Term Conditions in Primary Care SeĖngs of Haryana, India” is a mixed-methods,three-phase implementation study. It comprises (i) a formative needs assessment, (ii) co-design and development of adigital training module with CHOs, patients and clinical experts, and (iii) a pilot implementation evaluated using the RE-AIM and CFIR frameworks.

The project is funded by the National Institute for Health and Care Research (NIHR) General Health
