Our client is seeking a compassionate and experienced Bilingual Clinical Care Coordination Specialist to provide virtual care coordination and personalized support to high-risk members.
In this role, you will assess members’ clinical, behavioral, and social needs, coordinate care transitions and referrals, and connect individuals with appropriate healthcare and community resources. This role also requires fluency in Spanish and English, strong care coordination or case management experience, and the ability to collaborate effectively with members, families, healthcare providers, and community partners.
Schedule: Monday to Friday, 8:00 AM - 5:00 PM PST, with a 1-hour unpaid break
Responsibilities:
- Engage eligible high-risk members virtually to assess physical, behavioral, and social needs, obtain consent, and develop personalized care plans
- Coordinate care transitions, hospital discharges, and specialist referrals to ensure seamless, non-duplicated service delivery
- Connect members to vital community resources, social support services, and transportation
- Advocate for members and facilitate communication between families, medical providers, and community partners
- Support treatment adherence and empower self-management using motivational interviewing, trauma-informed care, and harm-reduction approaches
- Track clinical indicators, maintain accurate electronic health records (EHR), and support quality improvement (QI) initiatives
- Engaged members remotely to provide personalized support tailored to their environment and Managed Care Plan guidelines
Requirements:
- 1–2 years of experience in clinical care coordination, case management, or community resource settings
- At least 1 year of experience supporting seniors, individuals with disabilities, or those with mental health needs/challenging behaviors
- Bachelor’s or Master’s degree, or a valid professional credential/license in Case Management, Social Work, or Nursing
- Bilingual fluency in Spanish and English (written and verbal)
- Exceptional communication, counseling, organization, and record-keeping skills
- Current CPR certification (preferred)
- Proficiency with health IT systems and communication technology, with strong local knowledge of community resources (preferred)
Preferred Qualifications:
- Strong analytical and problem-solving skills, with the ability to interpret and display data visually
- Exceptional communication and interpersonal skills to collaborate with diverse clinical, provider, and community teams
- Proficiency in Microsoft Office (Word, Excel, PowerPoint) and basic digital design/charting tools
- Proven ability to work independently, manage projects, lead change, and execute quality improvement initiatives
