Help Build Knowsley's New Multi-Agency Team Supporting People Experiencing Multiple Disadvantage
Changing Futures is a national programme designed to improve outcomes for people experiencing multiple disadvantage, including homelessness, substance misuse, contact with the criminal justice system, domestic abuse and mental health. The programme aims to do more than support individuals; it aims to use learning from frontline practice to influence and improve the wider system.

The team will operate using relational, trauma-informed and strengths-based approaches, with a focus on building trusted relationships, reducing repeat crisis demand and improving long-term outcomes.
What Will the Team Do?
The team will provide intensive support to adults experiencing multiple disadvantage through:
- Small caseloads.
- Assertive outreach and engagement.
- Multi-agency problem solving.
- Adaptable working across organisational boundaries.
- Joint risk management.
- System navigation and advocacy.
- Identification of barriers within the system and opportunities for improvement.
