Role OverviewThe Patient Care Coordinator serves as the primary point of contact and advocate for patients accessing telehealth services, clinical research studies, specialty care for complex chronic conditions, and healthcare navigation support. This role demands exceptional empathy, communication, and organizational skills to ensure patients feel fully supported throughout their healthcare journey.
