**Care Coordinator - On-site (Chihuahua)**
**About the Role**
We are seeking **Care Coordinators** who are passionate about supporting dementia patients and their caregivers. In this role, you will provide high-touch care navigation using an AI-powered platform, helping reduce caregiver burden and improve patient outcomes.
**Key Responsibilities**
- **Onboarding & Relationship Building**:
- Enroll patients and caregivers on the platform and keep profiles updated.
- Maintain regular check-ins via phone, in-app chat, or occasional in-person visits.
- Build trusting, long-term relationships with patients and caregivers.
- **Care Navigation & Coordination**:
- Understand patient and caregiver needs and provide empathetic support.
- Act as the single point of contact for care needs and advocate for timely solutions.
- Connect patients to community resources and facilitate clinical referrals.
- **Platform & Data Management**:
- Use AI tools for note-taking, triage, and resource collation.
- Assist with in-home setup and troubleshooting.
- Analyze digital inputs and align data with clinical protocols.
- **Documentation & Compliance**:




- Document exclusively within the platform (HIPAA-compliant).
- Report issues and contribute feedback for process improvement.
**Qualifications**
- **Education**: Nursing or Social Work background.
- **Experience**:
- Caregiving experience required (dementia preferred).
- Familiarity with U.S. healthcare system and insurance.
- Minimum 3 years working with patients in a clinical or customer-service setting.
- **Language**: Fluent in English (Spanish is a plus).
- **Skills**:
- High empathy, patience, and strong communication skills.
- Ability to manage priorities in a fast-paced environment.
- Basic tech skills and understanding of HIPAA compliance.
- Nursing graduates or professionals passionate about patient care.
- Comfortable with technology and eager to learn new tools.
- Dedicated to making a positive impact on dementia patients and caregivers.
