**Job Description**:
**Responsibilities**:
- **Review Patient Cases**: Conduct daily reviews of all-well reports, prioritizing patients to identify urgent needs and plan effective care strategies.
- **Patient Communication**: Engage directly with patients to discuss their care plans, including mental health treatment, physical health management, and medication adjustments. Establish a compassionate and open line of communication to address patient concerns and needs.
- **Coordination with Healthcare Providers**: Act as a liaison between patients and a network of healthcare professionals. Responsibilities include facilitating access to mental health services, scheduling appointments with specialists, and ensuring seamless care transitions.
- **Filling Documentation**: Maintain accurate and comprehensive patient records, ensuring all patient information and updates are properly documented in the al-well system.
- **Administrative Tasks**: Oversee administrative functions related to patient care, including compliance with healthcare regulations and providing necessary data for quality improvement.
- **Breaks**:
Prioritize self-care through scheduled breaks to maintain personal well-being and professional effectiveness.
**Requirements**:
- Medical Degree from an accredited institution and a valid medical license.
- Excellent English level
- Excellent communication and interpersonal skills, with the ability to engage empathetically with patients.
- Strong organizational skills and the ability to manage multiple priorities in a fast-paced healthcare environment.
- In-depth knowledge of healthcare regulations, medical terminology, and documentation practices.
