Loma Linda Post Acute is seeking a driven, professional, and relationship-focused Admissions Director to lead our admissions process and serve as a key liaison between hospitals, referral partners, patients, families, and our clinical team.

Responsibilities

Manage and coordinate all admissions and referral activities for the facility.
Develop and maintain strong relationships with hospitals, case managers, physicians, and community referral sources.
Review referrals and collaborate with the interdisciplinary team to determine admission appropriateness.




Coordinate patient admissions, room assignments, and transition planning.
Meet with residents and families to provide information and facilitate a smooth admission process.
Maintain census development efforts and support occupancy goals.
Track admission metrics, referral trends, and marketing activities.
Ensure compliance with all facility policies and applicable healthcare regulations.
Partner closely with clinical leadership to promote quality patient outcomes and satisfaction.

Qualifications

Previous admissions, marketing, business development, healthcare sales, or case management experience preferred.
Skilled Nursing Facility (SNF) or post-acute care experience strongly preferred.
Exceptional communication and relationship-building skills.
Strong organizational and time-management abilities.
Ability to multitask and work in a fast-paced healthcare environment.
Proficiency with computer systems
