Project overview: The project involves a strategic transformation focused on modernizing care coordination capabilities for our client. As part of this initiative, a comprehensive platform is being deployed to streamline clinical workflows, improve patient engagement, and consolidate data across various care delivery systems. This endeavor is crucial as it aims to enhance operational efficiency and ultimately provide better outcomes for patients.
- Responsibilities: Lead the end-to-end implementation of Salesforce Health Cloud across care teams, clinical support staff, and administrative users
- Collaborate with stakeholders to define use cases, requirements, workflows, and success metrics
- Manage third-party vendors, consultants,



and internal Salesforce administrators/developers
- Ensure seamless integration with EMRs/EHRs, scheduling systems, and other healthcare platforms
- Define and enforce data security and HIPAA-compliance standards
- Drive adoption through change management, training, and support strategies
- Develop detailed project plans, track milestones, and report progress to executive sponsors
- 5+ years of Salesforce experience, with 2+ years specifically in Health Cloud implementations
- Proven experience in the healthcare industry (payers, providers, or life sciences)
- Strong understanding of care management, patient experience, and population health use cases
- Experience with integrations using HL7, FHIR, or Mulesoft
- Excellent stakeholder engagement and communication skills
- Strong leadership and project management capabilities (Agile or hybrid)
- Nice to have: Salesforce Certified Health Cloud Consultant
- Salesforce Administrator and App Builder certifications
- PMP or Scrum Master certification
- Experience with tools like Jira, Confluence, Smartsheet
