Life Steps Southern California Adult Services (LSF SoCal) seeks a Community Support Facilitator to provide home-based services to adults with developmental disabilities in Pomona, San Dimas, and West Covina. You’ll identify supports, develop person-centered plans, and teach life management skills.
Essential duties include training on mobility, transportation use, health and safety planning, and diligent billing/reporting. Must have a high school diploma or GED and dependable transportation.
