OverviewOur client is looking for a Lead Care Manager to play a pivotal role in driving clinical outcomes and coordinating comprehensive care for chronically ill or high-risk populations.In this position, you will serve as the central link between members, families, healthcare providers, and community resources within a collaborative, team-based environment.The adecuado candidate will successfully engage with a minimum of 25 members per day to deliver impactful case management services and foster meaningful health improvements.ScheduleMonday to Friday 8AM - 5PM (PDT Time Zone)ResponsibilitiesEngage eligible high-risk members in person or virtually to assess physical, behavioral, and social needs, obtain consent, and develop personalized care plans.Coordinate care transitions, hospital discharges, and specialist referrals to ensure seamless, non-duplicated service delivery.Connect members to vital community resources, social support services, and transportation.Advocate for members and facilitate communication between families, medical providers, and community partners.Support treatment adherence and empower self-management using motivational interviewing, trauma-informed care, and harm-reduction approaches.Track clinical indicators, maintain accurate electronic health records (EHR), and support quality improvement (QI) initiatives.Meet members where they are (homes, clinics, or community settings) in accordance with Managed Care Plan guidelines.RequirementsRequired Qualifications:Bachelor's or Masters degree,



or a valid credential/license as a Case Manager, Social Worker, or Nurse.12 years of experience in clinical care coordination, case management, or community resource settings.At least 1 year of experience supporting seniors, individuals with disabilities, or those with mental health needs/challenging behaviors.Current CPR certification (preferred).
Exceptional communication, counseling, organization, and record-keeping skills.Proficiency with health IT systems and communication technology, with strong local knowledge of community resources preferred.Preferred Qualifications:Bilingual fluency in Spanish and English (written and verbal).
