- Responsible to create patient profile in Infinite Database when Authorization for Initial Evaluation for MED A Only patient is received
- Responsible to enter all necessary data to create authorization for initial evaluation
- Data to be entered accurately and timely into Infinite Database for MED A cases and in Infinite Database as well as Raintree for MED B cases
- Process authorization based on sheet with patient information received from our partners/vendors
- Communicate appropriately and timely with team members
- Ensure that provider responds affirmative on receiving authorization with scheduled time for evaluation visit
- Follow escalation timeline for unresponsive providers as trained




- Cross trained in entering revisit authorization as well
- Provide coverage for other home care team members as needed
- Enter all necessary information in MED B tracker accurately and timely
- Participate in team meetings
