The Patient Care Coordinator is responsible for coordinating the patient experience from the

initial contact through ongoing care. This role schedules appointments, supports new patient

intake, coordinates referrals, maintains patient records, and provides excellent customer service

while ensuring confidentiality and compliance.

Responsibilities

● Answer incoming calls from patients, providers, and healthcare professionals.

● Schedule new patient consultations, follow-up appointments, and specialist referrals.

● Guide patients through the intake process and collect required information.

● Create, update, and maintain accurate patient records in EMR/EHR systems.

● Request, upload, and organize medical records and supporting documentation.

● Follow up with patients regarding appointments, referrals, and outstanding

documentation.

● Coordinate communication between patients, providers, and specialists.

● Ensure accurate documentation while maintaining compliance and patient confidentiality.

● Deliver a professional, compassionate,



and patient-centered experience.

Requirements

● Advanced English (C1+).

● +2 years of experience in customer service, scheduling, patient coordination, medical

administration, or healthcare support.

● Strong verbal and written communication skills.

● Excellent organizational and multitasking abilities.

● Experience using EMR/EHR systems or CRMs.

● Proficiency with Microsoft Office and Google Workspace.

Preferred

● Experience in healthcare, medical clinics, insurance, or telehealth.

● Experience with eClinicalWorks (ECW).
