Patient Care Coordinator (Bilingual – Spanish)
Schedule:
Monday–Friday, 10:00 AM–7:00 PM (1-hour lunch)
Position Summary:
We are looking for a highly organized, bilingual (English/Spanish) Patient Care Coordinator to manage daily patient communication, care coordination, and administrative workflow in our clinic. This role ensures timely and accurate information exchange with patients, case managers, and internal teams while maintaining excellent documentation and follow-through. Responsibilities may change based on operational needs, patient volume, and business priorities.
Key Responsibilities:
• Patient Communication & Coordination:
• Manage incoming patient, case, and referral communications (email, messages, internal systems) and route appropriately.
• Coordinate with internal teams for continuity of care and timely follow-up.
• Conduct outreach for follow-up appointments, procedures, and general care coordination.
• Send appointment information, reminders, and updates.
• Support communication with Spanish-speaking patients to ensure clarity and understanding.
• Administrative & Documentation Support:
• Process and organize incoming communications, identifying items needing action, escalation, or reassignment.
• Upload and maintain accurate patient and case documentation within the electronic health record (Epic).
• Assist with medical records and billing requests to ensure timely and accurate completion.
• Keep organized records of communication, case updates, and patient status.
• Support internal tracking systems and workflow documentation.
• Follow-Up & Workflow Support:
• Assist outreach to non-responsive patients or those pending scheduling or deferring care.
• Document communication attempts, patient responses,
and next steps clearly.
• Monitor assigned tasks for timely completion and appropriate routing.
• Coordinate follow-up appointments and procedures in line with clinic workflow.
• Team & Operational Support:
• Collaborate with Patient Coordination, Clinical, and Administrative teams for efficient operations.
• Adapt to changing priorities and operational demands.
• Provide general administrative and coordination support as needed.
• Assist with special projects or workflow initiatives as required.
Additional Expectations:
• Maintain professionalism, accuracy, and confidentiality in all communications.
• Follow clinic policies, documentation standards, HIPAA, and compliance guidelines.
• Demonstrate reliability, responsiveness, and strong organizational skills.
• Responsibilities may shift based on operational needs.
Required Skills & Qualifications:
• Bilingual in English and Spanish (required).
• Strong communication and organizational skills.
• Ability to manage multiple tasks with consistent follow-through.
• Comfortable in a fast-paced, team-based clinical environment.
• Experience with electronic health record systems (Epic preferred, not required).
• Prior experience in healthcare, patient coordination, administrative support, or call center preferred.
• Experience with personal injury or case-based patients is a plus.
