Help Build Knowsley's New Multi-Agency Team Supporting People Experiencing Multiple Disadvantage
Knowsley Council and partners are seeking applications from practitioners across the partnership to be part of the new Changing Futures Multi-Agency Team.
Changing Futures is a national programme designed to improve outcomes for people experiencing multiple disadvantage, including homelessness, substance misuse, contact with the criminal justice system, domestic abuse and mental health.
The programme aims to do more than support individuals; it aims to use learning from frontline practice to influence and improve the wider system.
We are creating a pioneering multi-agency team that will bring together professionals from different organisations to work differently around a shared cohort of individuals experiencing complex needs.
The team will operate using relational,



trauma-informed and strengths-based approaches, with a focus on building trusted relationships, reducing repeat crisis demand and improving long-term outcomes.
What Will the Team Do?
The team will provide intensive support to adults experiencing multiple disadvantage through:
Small caseloads.
Assertive outreach and engagement.
Multi-agency problem solving.
Flexible working across organisational boundaries.
System navigation and advocacy.
Identification of barriers within the system and opportunities for improvement.
Alongside direct work with individuals, team members will contribute to learning reviews, service development and system change activity.

Assessment Centre
If Successful you will be invited to an assessment day designed to reflect the collaborative nature of the programme.
Rather than focusing solely on interviews, the assessment will explore values, behaviours, relationship-building skills,



professional judgement and ability to work within a multi-agency environment.
