Position Summary
We are seeking a highly organized and detail-oriented Remote Care Coordination Specialist to support our U.S.-based multidisciplinary medical practice. This individual will focus exclusively on auditing patient cases, completing assigned actions, maintaining accurate documentation, and ensuring patient care workflows move forward efficiently.
This is not a direct patient care role. Success in this position depends on organization, attention to detail, accountability, and the ability to independently manage a high-volume workload while communicating effectively with our U.S. clinical team.
Primary Responsibilities
Case Audits
· Review assigned patient charts daily
· Audit referrals from receipt through completion.
· Ensure required documentation has been received.
· Verify referral status and identify workflow bottlenecks.
· Monitor patient progress through the care continuum.
· Ensure referrals are assigned appropriately.
Workflow Actions
· Complete assigned EMR actions.
· Follow up on outstanding referrals.
· Request medical records from outside providers.
· Track pending documentation.
· Verify authorizations when applicable.
· Escalate issues requiring leadership review.
· Document all work performed within the EMR.
Case Management Support
· Review inactive or stalled patient cases.
· Identify missing documentation delaying treatment.
· Monitor outstanding imaging, records, and consultations.
· Assist with second-opinion record collection.
· Ensure appropriate follow-up tasks are completed.
Communication
· Communicate professionally with internal staff.
· Contact medical offices to obtain records.
· Coordinate with referral sources when needed.
· Maintain timely communication regarding assigned work.
· Participate in daily huddles and team meetings.
Daily Expectations
· Audit assigned work queue daily.
· Maintain accurate documentation.
· Complete assigned action items before end of shift.
· Escalate barriers promptly.

Qualifications
Required
· Fluent Spanish
